Every volunteer must complete a waiver before volunteering with Western Lake Superior Habitat.  Please print and complete form and bring with you to the work site.

Standard Liability Waiver for Adults and Minors

For volunteer groups, Team Leaders should print out the Liability Waiver for Groups and have the entire group sign the form before the workday.
